Dr. Milchev graduated from the Medical University of Pleven in 1998. He worked successively as a resident physician at the Pleven Central Emergency Center from 1999 to 2002 and a general practitioner from 2002 to 2008, which helped him gain extensive clinical and organizational experience with patients with emergency and chronic diseases.

He worked as a medical representative for Novo Nordisk Pharma, where he built good communications with endocrinologists and patients suffering from diabetes mellitus from the region and the country. He obtained a specialty in endocrinology and metabolic diseases and in 2024 joined the team of the Clinic of Endocrinology and Metabolic Diseases at the University Hospital "Dr. G. Stranski" Pleven.

Dr. Milchev is a member of the Bulgarian Society of Endocrinology, Bulgarian Medical Union and the European Association for the Study of Diabetes (EASD).

His professional interests are in the field of diabetes mellitus - type 1 and type 2, modern insulin treatment, non-insulin therapies for type 2 diabetes, sensors for glucose monitoring, diagnostics and therapy in patients with insulin resistance and metabolic syndrome, osteoporosis (postmenopausal and senile), and thyroid diseases.
